Sick Notes
For a first sick note you will need a telephone appointment. Please call the practice on 01642 247029 to arrange this.
For repeat sick notes you can submit a request through our online consultation service -
A member of the team will review your online consultation and it will be dealt with in order of clinical priority. In some cases clinicians are able to diagnose/advise using a combination of the online consultation and your clinical record meaning you may not need to be seen in person.
Letters
Newlands Medical Centre can provide private letters. This is non NHS work and you will be charged a fee for private letters, please contact the practice to find out more information. As these letters are not NHS work they can not be asked for urgently as NHS work is our priority.
You can submit an online consultation to request these letters. To reduce delays in you receiving your letter, please ensure you have submitted enough information for us to process the request.
Alternatively, please request by attending Reception and completing a Consent Form.
Subject Access Requests
You will need to complete a form detailing why you require the records and what records you require. You will not be charged for a Subject Access Request (SAR) as long as the numbers of your requests are reasonable. Records are sent electronically to your email address so please keeps these records secure to avoid future unnecessary SARs. You will need to provide ID in person when requesting this information.
